Disability plates and placards are issued to a vehicle or individual upon submission of a completed DR 2219 Persons with Disabilities Parking Privileges Application (PDF)
. This form must be completed and signed by a physician for both plate and placard. Secure and verifiable identification of the person with disabilities is required. Parking privileges are available only to persons meeting the qualified criteria stated on the DR 2219.
Three types of registration are available:
- Permanent License Plates - Fees are calculated the same as a standard plates. The disabled person must be a registered owner of the vehicle.
- Permanent Placards (hang tags) - For persons with permanent disabilities who own more than one vehicle or do not own their own vehicle. The placard is to be hung from the review mirror inside the car, in which the disabled person is riding, while parked. This must be removed while the vehicle is in motion. The placard may be moved to any vehicle.
- Short Term Placards - are issued to persons with a short term disability for a period of up to 3 months.

- 1 set of plates
- 2 placards
- A set of plates and a placard
